The foundation of the gallery's collection was laid by Abbott Giuseppe Ciaccheri in the 18th century and has since been enriched by various donations and acquisitions. The Italian government assumed control of the museum in 1930, further cementing its national significance.

The second floor is dedicated to the renowned collection of gold background Sienese paintings from the 14th and 15th centuries, meticulously documented and displayed in chronological order. In 1977, the Spannocchi Collection was added, enriching the gallery with works by Northern and Flemish masters, thus expanding its scope to include significant international artworks. The ground floor diversifies the visitor experience even further with displays of cartoons, sketches, and sculptures, offering a broader perspective on the artistic process.

Prominent Sienese artists such as Sodoma, the Lorenzetti brothers, and Simone Martini are featured, highlighting their contributions to the art world and the cultural heritage of Siena.
